Full Job Description

We are looking for an enthusiastic and passionate Clinical Pharmacist to join our multidisciplinary team., The clinical pharmacist will operate within CRMP. Using and sharing pharmacy expertise to support GP's and the whole practice in effective medicines management and optimal patient care.

Undertake clinical medication reviews with patients, looking at medicine optimisation, prescribing and deprescribing recommendations.

Conduct patient medication reviews either on the telephone, face to face, or in a care home environment.

Possibility to undertake minor illness clinics, with the supervision/support of the GPs

Answer relevant medicine-related enquiries from GPs and other practice staff as well as patients with queries about medicines, suggesting and recommending solutions.

Providing follow up for patients to monitor the effect of any changes.

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Ensure awareness of quality and outcomes framework, PCN directed enhanced services, local enhanced services and work within those guidelines.

Ensure up to date knowledge of NICE guidelines and local ICB prescribing regulations.

Ensure patient notes updated professionally and thoroughly and correct coding is used for reporting purposes.

If you are motivated, with a desire for professional development and an interest in patient care we would love to hear from you., Cowley Road Medical Practice is a practice with 12000 patient population, situated in Oxford City means our cohort is very diverse; you will meet a wide variety of patients and cover a range of medical conditions in your work. While being a prescriber is not essential, previous experience in General Practice is desired, Your role will be predominantly patient facing and you will be given time for administration and audit work., Qualified and registered clinical pharmacist with minimum of 2 years post qualification experience, working either in public and/or commercial healthcare sector.

Have a good knowledge of GP general practice.

Have an appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing

Possess excellent written and verbal communications skills.
