We are delighted to recruit an additional Clinical Pharmacist to join our established PCN Pharmacy Team. We are looking for

This beautiful, green county is uniquely placed with providers across the system committed toworking together to improve the health and wellbeing of Herefordshire people and provide more joined up care closer to home.We are OneHerefordshire.Main duties of the job As a member of the multi-disciplinary team, you will be expected to provide prescribing advice and support to practice clinicians, staff, and patients. This includes prescription and medication queries, medicines policies and procedures, QOF and local incentive schemes, audit, and medicines safety. Clinical work at the individual practices will include medication reviews, minor illness consultations and reviews post discharge.

    WBC Primary Care Network includes 3 Hereford city - based practices; Wargrave House Surgery, Belmont Medical Centre and Cantilupe Surgery. As a small network, we are very fortunate to be able to work closely together to understand the shared needs of our patient population and directly implement new ways of providing excellent patient care. Primary Care Network staff are employed by Taurus Healthcare on their behalf. Taurus was established in 2012, as the provider arm of the GP Federation serving 185,000 patients in Herefordshire. Founded and owned by the partners of the entire Herefordshire Primary Care community, Taurus is focused on providing excellent out of hospital services for patients. Our ethos is to provide high quality and cost-effective health outcomes that are delivered as close as possible to the patients home, whilst ensuring that patients who do require in hospital services are seen as quickly and effectively as possible.
