We have 2 exciting opportunities that have arisen for enthusiastic and dedicatedClinical Pharmacist to join our practice teams and provide care for our patientsin the Tameside area. The Clinical Prescribing Pharmacist will have General Practice experience and be keen to be part of a largerorganisation as well as a smaller practice team. Our practice populations arediverse and multi-cultural with a strong community spirit. You will be part ofa busy and proactive team working alongside clinical and non-clinical membersof the team and the wider organisation. The successful candidate will participate in and support the Deputy Lead Pharmacist, medicines optimisation to deliver change for the benefit of patients and / or the organisation. You will be a key member of the senior clinical team being proactive in providing safer prescribing, providing expertise in clinical medicines advice, while addressing both public and social care needs of patients in their care. The role is pivotal to improvingthe quality of care, leading innovation and being aware of operationalefficiencies. The extensive footprint of the organisation can facilitate abroad depth of skills and experience to be developed in numerous environmentsand with the benefits of exposure to in and out of hours working. Main duties of the job Provide support to other clinicians with regards to prescription and medication queries / repeat prescriptions, and will demonstrate clinical reasoning and critical thinking, as well as the undertaking of comprehensive structured medication reviews and assessing / processing repeat prescription requests. Provide advice, care and medicines management for patientseither via telephone / video or face to face.
- Act as a positive role model supporting junior staff whist offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
- Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
- Ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload.
- Lead on the development of innovation bids, new services or a new mode of service delivery to meet the demands of the changing healthcare environment., access to gtd healthcares wellbeing initiatives, which offer a wide range of tools and resources;

- Liaises with ICB colleagues including ICB / PCN Pharmacists on prescribing related matters to ensure consistency of patient care and benefit.
- Liaises with ICB / PCN pharmacists and Heads of Medicines Management / Optimisation to benefit from peer support.
- Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to Patients, GP, nurses and other practice staff, other healthcare professionals including ICB pharmacists, pharmacy technicians, optometrists, dentists, health and social care teams and dieticians etc. Locality / GP prescribing lead, Locality managers, Community nurses and other allied health professionals, Community and hospital pharmacy teams and hospital staff with responsibilities for prescribing and medicines optimisation. Leadership :
- Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.
- Demonstrate understanding of, and contributes to, the workplace vision.
- Engages with Patient Participation Groups (PPGs) and involves PPGs in development of the role and practices where appropriate.

- Demonstrate understanding of the implications of national priorities for the team and / or service.

- Demonstrates ability to identify and resolve medicines related risk management issues according to policy / protocol, whilst understanding the process of escalation where appropriate.

- Demonstrate ability to extend boundaries of service delivery within the team. Research and Evaluation :
- Demonstrates ability to critically evaluate and review literature to ensure evidence based medicines and best practice are followed.
- Demonstrates ability to identify where there is a gap in the evidence base to support practice.
- Demonstrates ability to generate evidence suitable for presentations at practice and local level.
- Demonstrates ability to apply research evidence base into working place.
- Demonstrates understanding of principles of research governance. Other duties :

Excellent interpersonal, influencing and negotiating skills. - Excellent written and verbal communication skills.
- Good IT skills.
- Demonstrates accountability for delivering professional expertise and direct service provision.
- Work effectively, independently and as a team member., Mandatory registration with General Pharmaceutical Council.
- Masters degree in pharmacy (MPharm) (or equivalent).
- Specialist knowledge acquired through post-graduate diploma level or equivalent training / experience.
- Independent prescriber or working towards / intent of gaining independent prescribing qualification. Desirable
- Membership of the Royal Pharmaceutical Society (or equivalent).
- A member of or working towards Faculty membership with the Royal Pharmaceutical Society (or equivalent). Experience Essential
- Experience of working within primary care and / or general practice as a clinical pharmacist, with an understanding of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences ( patients).
- Able to obtain and analyse complex technical information and utilise data to improve clinical outcomes, including the review of medicine related incidents.
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
- Produce timely and informative reports.
- Gain acceptance for recommendations and influence / motivate / persuade the audience to comply with the recommendations / agreed course of action where there may be significant barriers. Desirable
- In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
- Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
- Able to undertake clinical audit and suggest changes of practice based on outcomes.
- Minimum of 2 years post-qualification experience.
