Full Job Description

Virtual wards support patients, who would otherwise be in hospital, to get the acute care, remote monitoring, and treatment they need in their own home, including care homes. Patients with a range of acute conditions will be supported to prevent unnecessary admissions and to reduce length of stay where the patient has been identified in the hospital, whilst maintaining the same level of support and care the person would have received in an acute bed.
The role of the Clinical Practitioner is dynamic, and the post holder will be highly proactive in continually developing and embedding the role beyond the traditional scope of non-medical practice, with the rest of the team. The post holder will work within locally and nationally agreed protocols and guidelines to provide assessment, investigation, diagnosis, evaluation, and treatment planning, supporting admission avoidance and facilitation of early hospital discharge for patients referred to our Virtual Ward Hub and supporting patients who require acute level care delivered within their own home environment.
Under the direction of the Virtual Ward Team leads you will assess patients on the virtual ward, implement a management plan, whilst considering your own clinical confidence and competence.
You will be part of the Virtual Ward service in a Multi-Professional team which provides high standards of clinical judgement, intervention and decision making, accessing the range of Community services., Demonstrate high level of judgmental/analytical skills when interpreting and performing patient assessments and care planning via the telephone and face to face.
- Maintain accurate and up to date patient care records, maintaining patient confidentiality, and statistical records as required by the service. Work with other professionals to promote multidisciplinary / multiagency working.
- Undertake clinical skills e.g Phlebotomy, ECG's, Physical Observations and monitoring.
-To attend and contribute to virtual ward MDT
- To work closely with the consultant clinical lead for virtual wards, community teams, and specialty teams in the acute to develop and support a range of virtual ward pathways.
- Working autonomously, assessing individual patient needs, initiating investigations, determining a differential diagnosis, and initiating appropriate, evidence-based treatment and care within scope of practice.
- Act as a resource for all members of the multi-disciplinary team, providing advice in aspects of patient management.
- Working with our clinical leads and integrated Multidisciplinary teams across organisations to develop the model for NHS@Home virtual ward universal personalised care within the Virtual Wards.
- Incorporating digital capabilities with the efficient use of technology to support clinical decision making, Responsible for the holistic assessment, planning, implementation and evaluation of evidence-based nursing care for patients referred to hospital at home service.
- Responsible for delivering, evaluating and supporting training of patient and carers enabling them to be cared for virtually.
- Utilising remote monitoring as part of the clinical care delivery
- Providing high-quality care for patients including co-ordination of the multidisciplinary team and allocating visits as appropriate.
- Delivering high-quality, clinically effective care, and patient experience, utilising research and in collaboration with colleagues in everyday practice.
- Participate in effective clinical leadership and management of team members through clinical supervision.
- Actively contribute to the development and improvement of services and the quality of care through reflective practice, clinical supervision and audit.
- Act as a role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff., Act as a role model, educator, supervisor, coach, and mentor.
- Actively seek and participate in peer review of own practice and participate in annual appraisals, demonstrating ongoing professional development by keeping up to date with national and local developments within the specialty.
Research
- Ensure that practice is informed by practice and may participate in research as appropriate.
- Evaluates own service or role within area practice but also across the key acute medicine and community pathways and engages or leads in quality improvement projects.
- Utilise results from research trails and clinical audit to develop new ways of working focusing on quality improvement and enhanced clinical care for acute medical patients.
- Evaluate and audit own clinical practice, selecting and applying valid, reliable methods, then acting on the findings.
Leadership
- Work with fellow colleagues to ensure that the team works flexibly and supportively together, and that the performance indicators for the area are delivered.
- Demonstrate excellent personal communication. Establish and maintain effective two-way communication channels with individuals and groups.
- Demonstrate appropriate assertiveness and ability to challenge others when the rights of patients and others may be infringed.
- Demonstrate leadership skills and ability to problem solve. Promote excellent team working and interdisciplinary relationships.
- Allocate work and assess performance of the team members objectively against set criteria/ competencies. Provide clear feedback to team members in a way that is conducive to maintaining and improving performance.
- Actively promote robust infection control practices. Assist the department Sister to maintain high standards of cleanliness in the clinical area and for a well-maintained clinical environment.
- Promote interdisciplinary working, with outcomes aimed at the best interest of patients.
- Actively promote diversity, including race diversity. Assist the department Sister to sustain relationships that promote dignity, rights and responsibilities. Identify and take action to address discrimination and oppression.
- Support the Senior Clinicians to ensure the clinical area practices within Data Protection/Confidentiality/Caldicott principles.
- Maintain accurate and contemporaneous records. To input and retrieve patient information from computer-based systems
- Organise, develop and deliver educational and training programmes, study days
