Full Job Description

The Clinical Review Team, review patients with 117 after care packages on behalf of the ICB. The team will review patients with learning disabilities, mental health and acquired brain injuries, so all level of registrations welcome., The Clinical Review Practitioner will hold a caseload of Patients with Section 117 after care needs who are in funded packages of care. The review will be in collaboration with the provider, the patient, and members of the MDT. The review will ensure the placement is ensuring financial efficiency and the patient is safe and is living in the most least restrictive environment. The review will also ensure the patient has the right legal framework in place to support their care delivery. The role will also include to lead on applying to Court for application and renewal of DOLS. Work alongside colleagues from contracting, performance and information to achieve the commissioner targetsCommunicate in a way that recognises difference and ensures that people feel included and their individual communication needs are met This will involve completion of the Reviewing documentand potentially other clinical tools for example from the risk management or safeguarding toolkits. Part of this would include presenting at the Quality Assurance panel to seek permission for funding for new placements, this would be achieved by completing the Joint Health Assessment and Social Care (JHAS)
