Clinical Risk and Safety Incident Co-ordinator

Support the clinical service lines (CSLs) to ensure that incidents that have resulted in moderate and above harm to a patient, or meet the Trust or National Priorities, have a robust SMART action plan which is then monitored through internal governance processes. Work with CSL teams to ensure that sharing and learning occurs regarding reportable events particularly where there are themes which can be shared which will improve safety across the organisation. Support events that take place, and processes that are in place to facilitate the learning identified from patient safety incidents is shared across the organisation. The post holder will support the Clinical Risk and Safety Lead and Clinical Risk and Investigations Manager with a caseload as part of an investigation team conducting reviews which may include a System Engineering Initiative for Patient Safety (SEIPS) approach into incidents reflecting the Trust and National priorities, in relation to PSIRF.
