Clinical Screener - Single Point of Access (SPA)

An exciting and challenging opportunity has arisen for a fixed-term position within the Integrated Independence Team for an innovative, enthusiastic and experienced administrative assistant to be a Clinical Screener within the Integrated Independence Team single point of access., We are looking for a Band 4 Assistant to coordinate the initial screening of all referrals into the service, as part of the Single Point of Access team. The role would also be well suited to an experienced member of a community therapies referral/co-ordination hub (or discharge hub). In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.,

• Together with the team of Clinical Co-ordinators, coordinate the referrals into the Single Point of Access team, screening all non-complex referrals and allocating cases to appropriate care pathways and IIT colleagues
• To provide first point of contact assistance/advice for referrers, health professionals and service users, forwarding callers to Single Point of Access team Clinical Co-ordinators as appropriate
• Liaising with primary, secondary, health and social care providers, to facilitate service users into appropriate Integrated Independence Team pathway or declining referrals where necessary
• Assess clients referred as appropriate to decide on appropriateness for the service
• To manage an individual caseload of service users with provision of therapy assistant assessment, treatment and liaison with Integrated Independence Team colleagues for advice or support as required.
• To update all electronic systems in conjunction with Single Point of Access Clinical Co-ordinators including referral email account, spreadsheets and RiO system.
