Full Job Description

This role is for a specialist Occupational Therapist to work within the Forensic Outreach service working with clients with complex mental health needs and forensic background as part of a full MDT in the community team of nurses, social workers, psychologists etc. This is an exciting opportunity to lead on social inclusion and occupational therapy whilst working as part of a close-knit team. The FOS team has a large, combined MDT, meaning a vast amount of shared experience, knowledge and points of view which contribute to robust and thorough case formulations and case reviews. The team is hard working and extremely supportive of one another with close working relationships to ensure support for each other in an otherwise demanding yet rewarding role., In this role you will be expected to facilitate/ cofacilitate community-based groups such as OT Recovery Through Activity, coffee group or creative writing group but even more so we want our staff to engage with service users and understand their interests and passions and make new groups or adapt current ones to suit them. Also requirement to carry out 1:1 work with service users, in community settings, hostels, at the hospital and in the service user's homes. This includes focusing on social inclusion work surrounding self-care, productivity (getting into employment or college) and leisure (participating in interests or group activities).
The team also includes an enhanced caseload which is a small, designated caseload of service users requiring more intensive, transitional and community input that sit within the larger standard caseload. With the enhanced caseload, the team are expected to see the service users multiple times a week in order to provide support and aid transition from hospital into the community., · To provide a specialist Occupational Therapy service to the Forensic Outreach service including clients with complex mental health needs and forensic background.
· To use evidence based, client centred Occupational Therapy assessments, interventions and outcome measures underpinned by Model of Human Occupational and the Recovery Approach.
· To be responsible for leading delegated occupational therapy and multi-disciplinary projects primarily within the Forensic Outreach Service and within other services as delegated
· To lead on the implementation of the clinical pathway of the service and audit,review and report on outcomes against set key performance indicators.
· To provide support and supervision on clinical work in the pathway for all members of the team as required.
· To provide leadership for junior Occupational Therapy and support staff across forensic community services, through supervision and appraisal.
· To work collaboratively and effectively within a multi-disciplinary community team liaising with inpatient services and community-based services as required
· To liaise with other Occupational Therapy and multi-disciplinary staff within the Trust and other services as required.
· To regularly supervise OT students on practice placement.
· To attend and contribute to relevant clinical meetings.
· To develop and maintain professional practice within the field of forensic mental health and Occupational Therapy.
