Full Job Description

This exciting role has been developed to support the urgent community response multidisciplinary team working across the South Tees locality. Your main role will be with the Home First Team who support early discharge from hospital when a package of care cannot be sourced. Part of your role will be to support with personal care and completion of activities of daily living. There will also be an element of therapy rehabilitation to improve the patients outcomes.

  • Numeracy and Literacy Level 2
  • Care certificate (willing to work towards if already holds NVQ level 3)
  • Level 3 vocational qualification orShort courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties and/or Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3;- Theoretical knowledge- Experience of a range of care and related procedures.- clinical observations- knowledge of relevant legislation
  • Completion (at the point of start date or within the probationary period) of the WASP competencies and Trust face to face training session related to insulin administration, and/or oral medication administration, following the Trust approved standard operating procedure and policy. Applicants must have access to a vehicle for work purposes to meet the requirements of the role., This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough, Redcar & Cleveland. Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post. You will need to be able to work on your own; as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed. You will work within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses. This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to help bridge the gap between hospital and home. As a senior integrated support assistant you will provide both rehabilitation and nursing care following a person centred plan within the patients' own home.
