Work as part of the team to deliver care in a reablement setting.

To support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.

To support patients being as independent as possible with their washing/dressing and nutritional needs.

To work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.

To support and deliver innovation within discharge practice and pathways.

To work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.

Support the delivery and co-ordination of care to a named group of patients.

Work proactively within the team to achieve unit and trust goals and objectives.

Work with and promote recognised policies, protocols, and guidelines.

As a Clinical Support Worker, you must live by our values and use your skills to ensure the ethos becomes part of the culture within the team. Being able to communicate well with a variety of people is an absolute must.

To carry out the instructions accurately and promptly of the reablement ward team who are professionally accountable for the delivery of care.

To work within an agreed reablement competency framework to support patients with their stated goals and within their functional capabilities.

To report to registered staff any patient that gives cause for concern or any observed changes in a patient's clinical condition.

To support patient's emotional well-being by supporting positive conversations, actively encouraging, and promoting independence on a range of activities of daily living.

To accurately document all care delivered to patients, including recording documentation electronically on trust systems always maintaining confidentiality.

To respond appropriately to clinical emergencies to ensure the patient receives appropriate care. Be aware of role in emergency situations and what actions should be undertaken.

To be aware of and adhere to all trust policies / procedures / nursing guidelines when undertaking any activities.

To make patients welcome to the clinical area and participate in the admission process under the supervision of registered staff.

Provide and deliver a high standard of individualised personal care (e.g.: washing/dressing and nutritional needs) with a reablement ethos.

To keep the unit clean and tidy maintaining a safe environment.

To adhere to trust infection control procedures.

To chaperone/escort patients to other clinical departments and or NHS Units under the direction of the registered nurse.

To support relatives under the guidance of the Multidisciplinary Team (MDT).

To assist with the discharge process for patients by the MDT.

NCH&C is hiring several multidisciplinary roles within the state-of-the-art new Willow Unit. Opening at Norwich Community Hospital in 2024, the new facility will provide a supportive and comfortable environment for 48 patients.

The restorative environment at Willow Unit will enable patients to make steady, step-by-step improvements and gain independence as they prepare to return to the community. Patients will be supported to play an active role in their own care and feel confident about performing daily activities when they leave. Everything down to the room décor, layout, and furniture at Willow Unit is designed to help patients get home sooner - there's even a supervised kitchen where patients can make themselves drinks and snacks.

Healthcare will be delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and wellbeing.
