Full Job Description

As Clinical Support Worker for Virtual Hub you will be responsible for providing a range of both clinical and administrative duties under the guidance of the team leaders in onboarding & supporting patients in the provision of remote monitoring & management of acute care in the patients' place of residence. As a support to the Virtual Hub, the Clinical Support Worker will be required to work independently performing several clinical duties/tasks following training and meeting competency within role as per policy and procedure. It is essential that you have phlebotomy experience & can travel to multiple appointments daily - if you do not mention both of these in your supporting statement, you will not be considered for the post.,

  • Working under the direction of a Registered Health Care Professional, participate in the clinical and administrative duties to ensure sustainable remote care monitoring and management provision for patients prioritising workload appropriately and developing competencies that safeguard practice and effectiveness in role
  • Have proficient attitudes and behaviors compatible with NHS and Trust Values
  • Work as part of a designated Virtual Hub team delivering care that focuses on the remote needs of the individual
  • Support with the organisation of remote monitoring equipment delivery and equipment, working across the trust localities and performing home visits.
  • Developing positive relationships with partners to the Virtual Hub Service, in particularly the Virtual Hub Volunteers.
  • Be aware of the patient's needs - emotional, social, physical, and cultural. Respect privacy, dignity and beliefs. Acts in a way that supports equality and diversity values.
  • Work as part of the team in a co-operative, professional and constructive manner.
