Full Job Description

You will work as a Team Leader in the Crisis Response Team. The post will involve completing crisis complex mental health assessments and being a point of reference for advice and guidance to junior staff who require support with complex cases.
You will have responsibility for the day-to-day supervision and management of lower banded staff within the Crisis Response Team. This will include the management of these staff in line with agreed Trust HR processes.
The Crisis Response Team is one of three teams that provide crisis and urgent access to secondary mental health services. You will be expected to work closely with the Band 7 Team Leaders from the other teams that make up the Urgent Care Pathway - Hospital Liaison and Home Treatment teams - in order to ensure effective service provision and compliance with Key Performance Indicator targets
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts where appropriate and operationally required. Participation in projects to review and develop service provision is a requirement for this post., + Provide comprehensive assessment and care planning in line with the requirements of the Mental Health Care Clusters
+ Produce accurate and timely documentation on all service users in line with your professional code of conduct and Trust policy.
+ Triage of referrals received and offer advice to other clinicians to ensure a timely response and accurate allocation of referrals received according to risks.
+ Participate in the planning and ongoing review of service users either awaiting assessment or allocation for continuing care to ensure care provided is effective and safe
+ Provide practical support, advice, signposting and education to service users, carers and other disciplines.
+ Have an awareness of other statutory and voluntary service provisions to signpost service users for appropriate care.
+ Responsible for the recording of all patient related activity information onto Paris.
+ Carry out risk assessment of the individual and of the potential hazards in the service user's environment.
+ Chair multi-disciplinary meetings i.e. referral / review / business., In addition to core clinical and non-clinical support, the following are additional key activities identified as being necessary to the role in the delivery of your normal day-to-day activities.
+ Complete mandatory training in line with the Trust Policy.
+ Receive annual appraisal and conduct annual appraisals to lower banded staff.
+ Provide training relevant to lower banded staff.
+ Take all allocated annual leave.
+ Receive management supervision.
+ Comply with the CPD and practice hours requirements set out by Nursing Midwifery Council NMC / Medicines competency framework (This is the essential requirement for yearly registration)
+ Medicines competency framework.
+ Mentorship and supervision of students and lower banded staff.
+ Receive clinical supervision and provide clinical supervision to Band 6 clinicians
+ Participate in SI investigations, disciplinary and grievance procedures plus any relevant investigations or scrutinising of notes in line with Trust policies.
+ Deliver clinical supervision to lower banded staff in accordance with Trust policy.
+ Attend seminars / conferences and other CPD events - external/internal.
+ Attend regular key Trust-wide meetings / forums i.e. Professional nurse advisory committee, nurse forum, Link nurse forum (Infection control)
+ Attend additional in-house developmental training.
+ Be a Paris representative.
+ Act as Safeguarding Champion.
