An exciting opportunity has arisen within Calderdale ICB Continuing Healthcare Team for a part time Childrens Clinical lead nurse, working 18.75 hours flexibly to meet the needs of the service. The successful candidate will be working remotely and will be provided with all the technical equipment to fulfil the requirements of the role.

The candidate will demonstrate an understanding of working in the community, hospitals, working with families as well as a multidisciplinary, multi-agency environment; be familiar with children's complex health needs and Childrens Continuing Care.

We are looking to recruit an individual with excellent partnership, networking and communication skills who has strong collective leadership skills that can motivate individuals and teams to build and sustain change.

This role will include working with Children age 0-25 transitioning into adulthood., Be the line manager for the Childrens Case Management Service (0-25). Providing leadership and support to the Case Manager with the day-to-day operational management of the service.

maintain the efficiency and effectiveness of the service in line with the National Framework for Children and Young Peoples Continuing Care.

Adopt a child centred approach to working with a range of people across cultures in both community and hospital settings.
