Full Job Description

The post holder, in addition to providing clinical input in the assessment and case management process for Children and Young Peoples (0-25) Continuing Care as part of a wider management team, will be responsible to undertake the following: To provide clinical supervision and complete assessments to determine eligibility in regard to the National Framework for Continuing and Young Peoples Continuing Care. To be highly knowledgeable regarding the National Framework for Continuing and Young Peoples Continuing Care and on the roles and responsibilities of the ICB within this process. To influence and can challenge decisions regarding the implementation of the continuing care process. To use clinical expertise in determining appropriateness of a care package to meet an individuals needs whilst assuring the quality and effectiveness of that package. To ensure that the individual care packages are reviewed in line with local policy. To proactively monitor and manage the workload of the team
within agreed local and national targets and keep the Senior Clinical Operational Manager updated. To effectively manage a small team, reviewing performance of individuals and team. In discussion with the Senior Clinical Operational Manager, set individual and team performance targets. Will support the Head of Service and Senior Clinical Operational Lead and their team to ensure that their work-stream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery: Work alongside the Business Manager and assist in the Management of the Teams business support function, including developing and managing the teams business plan, progress and reporting risk and issue management. Liaise with appropriate functions within the ICB. Manage all corporate business returns, including planning and workforce returns. Develop and implement a content management system to ensure information is properly managed and best practice is shared across the team,
Directorate and ICB Take a lead in the coordination of training & development and recruitment activity across the Team. Oversight and management of all aspects of the Team budgets where delegated. The above is only an outline of the tasks, responsibilities and outcomes required of the role. The job holder will carry out any other duties as may reasonably be required by their line manager. The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the Department and the ICB Will line manage a team of Childrens Continuing Care Nurse Assessors responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures. Will ensure that they and their team always adhere to the relevant National Framework and guidance, NMC Code of Professional Conduct and wok within established policies, procedures and systems using quality standards and performance as directed
by the Operational Lead. Will support the Senior Clinical Operational Lead to ensure that their work stream is planned and managed effectively and take the lead in their successful delivery. Will ensure compliance with all responsibilities associated with the process and protocols locally and nationally with regards to the National Framework for Children and YPs (0-25) and any related legislation including Personal Health Budgets Work in partnership with all professionals to develop and maintain effective communication and good working relationships with ICBs, Primary and Secondary Care and Local Authorities Will assist in the development of a positive professional and working relationship with the Customer/s
