This is an exciting opportunity to lead a newly forming team
The post holder will promote effective relationships with a range of key partners and stakeholders by working collaboratively to ensure servicers are fit for purpose in meeting the needs of the community now and into the future
The post holder will be responsive to the evolving and changing needs of the population, the workforce, and the NHS. Utilising a dynamic and pragmatic approach to problem solve and consider the best approach & solutions to complex matters., The post holder will operate as part of an integrated multi-disciplinary team and be flexible in terms of travel to undertake duties. The team are currently based in Huyton but this may change due to the team expansion.
The post-holder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care, and the setting of standards of care within that team.
The post holder will be aligned to CERT and may be required to work flexibly across all divisional sites to provide response to the service needs of the physical and mental health clinical teams providing cover as directed by the Service Manager to ensure continuity of a quality service provision.

You will be required to enrol for this service for a fee of £13 per year and maintain registration.
