If you are an innovative and forward-thinking Nurse who is ready to take on an exciting Clinical Triage role, then this post is for you!An exciting opportunity has arisen for an experienced Band 6 nurse to join a multidisciplinary team working within our Integrated Single Point of Access (ISPA). Your role will be to triage patient referrals that come into Coventry Community Physical Health Services and ensure our patients get the right care from the right team at the right time.As an experienced Clinician, you will apply your clinical knowledge and risk assessment skills to triage and clearly identify urgent and non-urgent patient referrals and allocate and arrange visits to our patients within the community setting. The role is predominantly completing telephone triage so it's essential you have excellent communication skills as you will regularly be liaising with patients, carers and other professionals. You will also be IT competent and confident as you will be working with technology
and multiple patient information systems.As a valued team member your development will be supported through a robust induction program, a values based appraisal, one to one meetings, regular in-service training and clinical and managerial supervision. Training opportunities are supported by our Practice Based Facilitators and is actively encouraged by Team Leads.
o To have continuing responsibility to prioritise referrals through the integrated single point of access (ISPA)using sound clinical knowledge and risk assessment skills to ensure patients are transferred to the most appropriate service to meet their health care needs.o To maintain knowledge, skills and attitudes necessary to deliver an evidence-based service that is responsive to the changing needs of the population and to ensure all contacts with the ISPA service reach an effective and appropriate outcome for the patient.o To participate in peer to peer supervision, act as a senior resource and support with other team members.o To be responsible for the provision of effective learning experiences for pre- and post-registration studentso To effectively coordinate the resources available within Community Sevices to maximise access to services or sign post to other services available.
