Full Job Description

The Learning Disability service receives many referrals every day. We are looking for an experienced Practitioner (30 hours per week) to help provide an efficient triage of the referrals to ensure all referrals are handled in a timely and equitable way., Clinical triage of referrals received by Learning Disability services to determine if the referred patient is eligible for DPT LD services under the current eligibility criteria.

Liaising with stakeholders for each patient (care providers, GPs, families etc) to briefly assess the patient's level of Learning Disability, and to ascertain if LD services have consent to share information and provide treatment.

The role has the responsibility of decision maker for whether or not a referral is accepted to LD services.
