Full Job Description

The role of colorectal cancer support worker will be to work with the Colorectal clinical nurse specialist to support patients undergoing a colorectal cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer., · To Support the Colorectal Nursing Team in the delivery of nursing care and services to patients from pre-diagnosis to end of life care in primary & secondary care at any point of their colorectal cancer journey;
· Provide support to the Colorectal Nursing Team, contributing to Multi-disciplinary Team (MDT) discussion, patient assessment and reviewing care plans established by Colorectal Nurses and wider MDT;
· Providing support with clinical administration, including collating expediting results and supporting the Cancer Pathway Co-ordinator with the organisation of MDT discussion lists;
· Supporting the Colorectal Nursing Team with activities undertaken to raise awareness of the signs and symptoms of colorectal cancer;
· Supporting the Support Group and Health and Well Being Events as required;
· Assisting with the day to day running of the service including providing information for patient and carers and where appropriate sign posting to most relevant service., · To demonstrate a professional approach to work and promote positive relationships with other departments with a multidisciplinary approach across primary and secondary care;
· Participate in meetings as requested by the Colorectal Nursing Team in secondary or primary care;
· To maintain confidentiality at all times in line with Trust and Data Protection policies;
· To be aware of and comply with all requirements for Health & Safety procedures and ensure Trust policies and procedures relevant to the area of practice are adhered to.
Clinical
· Work collaboratively with the Colorectal Nursing Team to provide a seamless service which is responsive to individual patients needs;
· Ensuring timeliness of investigations with appropriate follow up appointments based on clinical need as assessed by Colorectal Nursing Team;
· To provide advice and information with regard to aspects of investigations to patients, carers and other health professionals;
· Liaise with all members of the extended MDT to aid facilitation of a collaborative approach to the care of the patient;
· Ability to promote patient centred care and establish good relationships with patients and significant other;
· Communicate effectively with the patient and significant other to ensure they understand all aspects of care delivery;
· Carry out all aspects of care as requested by the Colorectal Nursing Team;
· Ensure all aspects of care are accurately documented in line with Trust policies & procedures;
· Ensure the environment is safe for staff and patients for the delivery of care;
· Place patients on MDT discussion list as requested by Colorectal Nursing Team;
· Act as a point of contact for patients /significant other / Health Care Professionals in addition to the Colorectal Nursing Team;
· Attend clinics as required to support patients having investigations;
· Ensure knowledge and skills are continually updated to assist in the development of service delivery;
· Identify and make contact with patients during their cancer journey, identifying required level of intervention under supervision of the Colorectal Nursing Team;
· The Colorectal Cancer Support Worker will be expected to, under guidance and using strict protocols, take on some aspects of the patient management.
Administration
· Triage incoming calls using good communication skills and basic clinical awareness, liaising as appropriate when non routine, and refer complex decisions to the Colorectal Nursing Team for assessment and review;
· Co-ordinate necessary assessments, appointments or investigations to fast track people back into the system as required;
· Highlight potential delays in investigations, diagnosis and treatment throughout the patient pathway and escalate appropriately;
· Assist Colorectal Nursing Team in relevant service audit and research;
· Ensure appropriate databases are regularly updated;
· Assess and allocate timely and appropriate Colorectal Nursing Team Survivorship follow up appointments as required;
· Ensure electronic, faxed, verbal communication is sent to primary, secondary, tertiary centres in a timely manner;
· Order educational leaflets to support the Colorectal Cancer Service;
· Order supplies for nurse led clinics.

GOVERNANCE
· Maintain systems and processes to establish and maintain effective communication, and confidentiality of information;
· Assist with the maintenance of systems and processes to continually monitor standards, e.g. benchmarking, audit, and implement action plans to improve quality of services;
· Support the Surgery and Cancer Division to ensure that lessons are learnt from issues arising from risk, reports, complaints etc. and that they are promptly and positively addressed;
· Contribute to the development and implementation of policies, procedures and guidelines related to own service;
· Ensure compliance with Trust policies, procedures and guidelines, taking action / alerting the Divisional Senior Management Team if practice appears to contravene policy, or if concerned about any aspect of patient care.
MANAGERIAL/LEADERSHIP
· Act as a role model by demonstrating leadership and expertise, and by maintaining credibility across the Trust, the wider health care community and external agencies, ensuring a positive image of the Trust is maintained;
· Participate in meetings internally or externally to the Trust, relevant to specialist area of practice, proactively contributing to the meeting by providing advice and expertise in own specialist area.;
· Support the Division in the effective use of resources, e.g. staff, budget, premises, equipment, supplies and materials;
· Maintain systems and processes to promote a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns., · Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning;
· Assist with systems and processes to ensure that learning needs are identified and met and that all learning is planned, implemented, evaluated and shared in order to change and improve services according to changing health care needs.
