Full Job Description

The post holder will be required to deliver patient care complementary to that offered by Doctors and other health care professionals within the field of Stoma / Colorectal Care as part of a multi disciplinary team. The successful post holder will act as part of the colorectal care nursing team and support point both in-patient and out-patient services to ensure patients receive appropriate management/treatment, aide their pathway and contribute to decreased length of stay.
The post holder will work collaboratively as a member of the Stoma / Colorectal team to ensure both elective / emergency patient receive holistic assessment, planning, implementation and evaluation of their care needs.

Are you an enthusiastic and highly motivated Nurse who is ready for a challenge, the East Site Stoma / Colorectal Nursing team is seeking a highly self-motivated practitioner, with a desire to promote and develop clinical excellence in clinical practice. + Post Registration Education, a Diploma / Degree Level
+ Post-Registration nursing experience at Band 5 level or above
+ Demonstrate involvement in change management and successful implementation of practice / quality improvements.
+ Excellent Organisational and Leadership skills.
+ Experience of multi professional working / working effectively with other stakeholders
