Full Job Description

This is an exciting opportunity for an enthusiastic individual that drives towards health promotion and improvement. The post holder will work closely with the multidisciplinary team to ensure that all aspects of the Enhanced Recovery (ERAS) is optimised in the Intermediate Surgical Care Unit (ISCU) and surgical wards for all patients on the programme. This includes pre op clinics discussing ERAS with patients and prescribing the pre op medications, day of surgery checks and patient support post op ward rounds and supporting the medical staff with the outcomes, daily monitoring of the ERAS patients offering guidance, support and ensuring all outstanding requests are acted upon and reported. Discharge planning and ensuring discharge letters are correct and completed in a timely manor to prevent delays. This role continues post discharge giving the patients a link to the service for up to 10 days where they receive follow up phone calls. This is to ensure the patient is progressing and offered advice where necessary re analgesia, diet, wounds escalating when appropriate., o Responsible for working with multi-disciplinary teams to support implementation and ensure the development of Enhanced Recovery pathways in the hospital o To actively promote Enhanced Recovery Programmes and the benefits that can be achieved through implementing them. oIn collaboration with the clinical teams provide training programmes, for staff and patients in the Trust, related to Enhanced Recovery programmes. oBe able to present at local meetings on relevant topics related to Enhanced Recovery oTo join on ward rounds with senior surgical teams, be a patient advocate, and help deliver the Enhanced Recovery elements by advising appropriately oSeeing patients on the programme on the wards and Intermediate Surgical Care Unit (ISCU) to ensure Enhanced Recovery elements are being achieved where possible. oAble to work autonomously and be part of a team. oHolding one to one Enhanced Recovery Clinics as part of a one stop clinic pre op to ensure optimisation of the patient, working closely with the Stoma Specialist Nurses including stoma siting. oSupport the patient post discharge via telephone clinics, to ensure the patient is progressing, giving advice where necessary re analgesia, diet, wounds escalating when appropriate. oAchieve the goal of delivering gold standards of care helping to reduce length of hospital stay and readmission rates o To collect, input patient data and audit outcomes to evidence possible changes to patient surgical care
