Full Job Description
An exciting opportunity has arisen in the colorectal nursing team for a stoma care nurse to cover maternity leave for a fixed term contract. This position is for 30 hours per week. The team would welcome informal visits to . The team work over all OUH sites, providing high quality stoma care including community clinics and home visits, providing exceptional patient care which is underpinned by evidence based practice and OUH values. In partnership with the patient and their carers and are actively involved in the caring process. Manage, direct and evaluate the nursing care for a defined caseload of patients. Communicate effective information to patients/relatives/carers and all members of the multidisciplinary team. Developing and maintaining specialist knowledge of nursing practice relevant to the clinical area. Contributing to continuing professional development in the department and mentoring students and other professionals. Working proactively to ensure every hospital visit meets the needs of the patient, ensuring effective communication with the patient/family and other agencies., Be a defined point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice and psychological support. Be responsible for the assessment, planning, implementation and evaluation of patient care, demonstrating a collaborative approach through effective communication and decision making between the multi-professional team, patients and significant others. Work collaboratively and in partnership with other health care professionals, offering advice to colleagues. Accurately document and report to other members of the nursing team and multidisciplinary team information regarding the care given to patients, communicating changes as they occur. Identify and discuss patient needs with the team, patients, carers and the multi- disciplinary team. Be able to plan own workload and those of others as required. Assess health and psychological needs of individual patients and provide care to meet these needs, through a process of shared decision making. Demonstrate a problem solving approach to effectively execute planning, implementation and evaluation of individualised plans of care based on a person centred model of care. Communicate complex and sensitive information effectively to patients, carers and other staff, overcoming barriers to understanding and providing support during distressing or emotional events,

- Accept patient referrals to agreed criteria and be identified as the key worker for an agreed patient/client caseload.

As negotiated with Senior Specialist Nurses, set and maintain standards of practice, conduct and decision making. · Be competent in the use of all relevant Trust information technology ( IT) systems including the use of electronic patient records, (EPR), email and associated professional media ensuring that data is entered in an accurate and timely fashion, in accordance with Trust policy and procedure. · Engage in and promote a paper light approach to information management. · Maintain the use of email and other modes of communication in order to keep abreast of information disseminated across the Trust. · Demonstrate an awareness and understanding of Accountability, Responsibility and Duty of Care and work towards the provision of support and education for less experienced and non-registered staff on the implications for practice.Management and leadership
- Adhere to NMC Code of Conduct maintaining patient confidentiality at all times · Promote effective interpersonal relationships with all members of the multi-professional team thereby ensuring a good team spirit and collaborative and person centred working practices. · Be involved in developing standards of care, practice guidelines and where appropriate Care Pathways for the colorectal patient/client group. · Be proactive in undertaking specific responsibilities as delegated by the senior specialist nurses. · Contribute to the development and delivery of nurse led initiatives for the benefit of colorectal patients, in conjunction with medical and nursing/AHP colleagues. · Demonstrate and share clinical practice developments to provide evidence based care and contribute o the service improvement in accordance with local policy initiatives. · Contribute appropriately to clinical governance activities that relate to own area of practice and patient/client group.,

- Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through self education and attendance at relevant courses of study.

- Contribute to the development of specialist nursing knowledge and skills to enhance and extend the practice of other nurses and health care professionals within the service.
- Identify and address the educational needs of patients, families, carers and staff involved in the delivery of this service.
- Provide specialist nursing support, information and education for patients, families and carers who use the service.
- Contribute to the teaching and support of students from the multidisciplinary team within relevant educational programmes
- Organise and contribute to relevant educational programmes and in house study days.
- Develop and maintain a peer network of support, information and learning with other specialist practitioners at al local and national level.
- Maintain ones own professional portfolio and development in line with NMC .
- Participate in multidisciplinary education and training. · Supervise, teach and support the nursing and non-nursing staff involved in direct patient care, ensuring they have the necessary knowledge and skills to undertake the planned care, within a conducive learning environment. · Undertake all relevant Statutory and Mandatory (SM) training using the Trust E-Learning packages and study opportunities. · Undertake the responsibilities of a mentor participating in induction and orientation programmes, guiding practice, assessing competence, enabling and supporting learning Improving Quality and Developing Practice · Comply with post registration education and practice (Prep) requirements for NMC Revalidation or equivalent, identifying own learning needs and ensuring own professional development is maintained by keeping up to date with practice developments. · Demonstrate a commitment to evidence based practice and clinical excellence. - Registered Nurse (Part 1. 8. 12 or 15 of the Nursing and Midwifery Council Register depending on area of practice) or National Certification in area of Practice.
- UK recognised Degree in Nursing relevant to post.
- Evidence of on-going professional development through maintenance of an NMC revalidation or equivalent.
- Minimum 3 years experience in a related area Desirable criteria
- Evidence of a career development plan including higher level Nursing Qualification, Able to demonstrate up to date knowledge of nursing practice on issues relating to colorectal nursing
- Ability to work under own initiative and within boundaries of the role
- Good interpersonal, organisational & management skills.

- Experience of working with colorectal cancer/stoma patients, Able to demonstrate the ability to put patients and their families and carers at the heart of what you do and recognising different needs by encouraging a spirit of support, integrity, respect and teamwork.
