Full Job Description

As a key member of the specialist multidisciplinary team you will support patients through their diagnostic and treatment pathway responding to patient care and support patient needs using an holistic assessment framework, mostly in the hospital setting with occasional community input. This involves supporting patients and their families in the out patient setting who have been given a cancer diagnosis, incorporating holistic needs assessments as required and carrying out telephone support. As part of the patients colorectal pathway you will be required to teach and support some of our patients on the wards to become independent with their stoma care and then provide follow up in nurse led stoma clinics., As a colorectal/stoma nurse you will manage a caseload with support and leadership from your team leader. You will work collaboratively with colleagues and liaise with healthcare professionals both within the hospital and community setting.

Training and education will be provided to develop and build on your existing knowledge.
