West Herts Community Heart Failure Service: Band 7 Specialist Nurse/Clinical Pharmacist/Paramedic - 6 month Fixed Term Sabbatical Cover Would you like to be at the forefront of delivering new models of care through transformation that meets the needs of the population? Would you like to work with a team who will support and develop you? If you can answer yes to any of the above then we may just have the job for you! We are looking for an experienced Nurse, Paramedic or Clinical Pharmacist with advanced clinical skills, who is driven, enthusiastic and innovative with experience and expertise in the care and management of patients with cardiac conditions. You will be joining our existing and well-established Heart Failure Team within a community setting and with close links with our acute partner, Cardiac Rehab and Palliative Care Team. The team is also well supported by a consultant cardiologist and administrators In addition to managing a caseload of heart failure patients in their own homes, virtually or in a clinic environment, you will support the Heart Failure Virtual Hospital, and support the implementation of innovative pathways to transform the way Heart Failure patients receive care across West Hertfordshire., Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010) For further information please contact: Zoe Jayne Clinical Lead - Heart Failure and Cardiac Rehab Service (Hertfordshire Division) [email protected], The post holder will be expected to develop and hold an appropriate caseload of heart failure patients. Utilise specialist skills to assess clinical condition and interpret and analyse results of investigations Identify appropriate patient requiring support from the Heart Failure Virtual Hospital Identify appropriate patient requiring IV diuretic support Provide expert advice, guidance and information to health and social care professionals and other non-professionals both within and external to the organisation. You will be an experienced and committed nurse, paramedic or clinical pharmacist with cardiac experience who is motivated and energised to take part in innovation to change the way services are delivered to improve patient outcomes. The role involves working closely with GP's, other members of the integrated community teams, and with social care partners. As such you will be required to have excellent communication skills and be confident to work using your own initiative as well as being an excellent team player.
