Join Bromley's integrated primary and community transformation team to be a key part of designing and delivering our whole-system model of care, in ine with One Bromley's strategic plan. Like the wider NHS, Bromley faces growing demand for health services in the future; transformation and improvement is required to ensure the sustainability of primary care and the healthcare system as a whole.

We are seeking a pivotal role in our team to support primary and community care improvement across Bromley. The postholder will use data and information to develop insights about primary and community services, monitor contract delivery, design and produce communications for a range of audiences, including GP practices, wider health and care providers and the public, lead the implementation of projects and coordinate complex requests on behalf of the team.

The successful candidate will be able to demonstrate their knowledge, technical and analytical skills as well as excellent communication skills, both written and verbal, and an ability to build strong working relationships across the health and care system. They will have a flexible, delivery-focused approach to resolving issues and finding solutions to problems.

This post is based within a friendly, dedicated and energetic integrated team dedicated to improving health services for people in Bromley. The postholder will be expected to work in the Bromley office regularly, in line with the ICB's advice to staff, as well as visiting health care providers across the borough.

The South East London Integrated Care System (ICS) brings together the health and care partners that serve our vibrant and highly diverse populations resident in the London boroughs of Bexley, Bromley, Greenwich, Lambeth, Lewisham and Greenwich - our Places.

Our partnership brings together six local authorities, over 200 general practices (operating within 35 Primary Care Networks), Guy's and St Thomas' Hospital NHS FT, King's College Hospital NHS FT, Lewisham and Greenwich NHS Trust, South London and the Maudsley Mental Health FT and Oxleas FT. Importantly, the ICS seeks to be connected to the communities we serve (circa 1.92m residents) and work with the widest possible range of community, voluntary and third sector groups and organisations in each borough. The reach of our NHS provider portfolios extends beyond the borders of the ICS, across London, the south of England and nationally for some services.
