Apply now and come and join our skilled and friendly team on this thrilling journey and help reshape the future of Continuing Healthcare across the Cheshire footprint., As a Commissioning Practitioner, you will be responsible for assessing individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is adhered to. This also includes planning and commissioning packages of care and the review of these to ensure they meet identified health outcomes.
You will support personalised care by offering personal health budgets and help individuals and their families to understand these.
Successful candidates will have excellent communication skills, be resilient and have the ability to work autonomously and as part of a team.

We have posts available in three locations / bases Chester, Crewe, and Macclesfield.
Part time applications will also be considered.

Detailed job description and main responsibilities

The post holder will:
+ ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the ICB within the service's key performance indicators.
+ commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
+ provide case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care.
+ manage complaints/disputes.
+ process urgent fast track referrals and, where appropriate, procure packages of care in a timely manner for individuals who are receiving or require end of life care.
