To prevent unnecessary hospital admission and facilitate earlier supported discharge. To provide advanced clinical assessment for patients in various locations for example, their own home including care home and intermediate care beds. As a member of the intermediate care services the post holder will work as a Community Advanced Clinical Practitioner within the multi-disciplinary team., Within the Urgent Community Response and Virtual Ward model including the IV pathway, the post holder will provide advanced clinical assessment to patients with long term conditions including a focus on frailty and provide care programmes appropriate to the rehabilitation and anticipatory planning needs of individual patients. The post holder will work in partnership with the Community Consultant Clinical Practitioner, Lead Community ACP, Team Leaders, Service Managers, Therapists and other staff within intermediate care and or community teams.
The post will work in partnership with the Consultant in Elderly Medicine, GP, Community Matron, Case Managers, social care staff and the wider multi-disciplinary team to provide personalised care across intermediate tier services and community services in the community including the voluntary sector.
