Full Job Description

An exciting opportunity has arisen to join Bolton's Adult South District Team as a Community Assessment Officer. You will collaborate with social work and health colleagues to support adults and carers eligible for assessment and care services under the Care Act 2014.

Our team in Bolton, which includes Social Workers, Occupational Therapists, Nurses, and Mental Health Practitioners, aims to help people live well at home. We focus on person-centred, strength-based practice to provide quality health and social care, protect the vulnerable, and foster self-sufficient communities.

The role involves assessing and supporting individuals with care needs, addressing complex issues, family dynamics, legal matters, crises, and coordinating with other professionals as required.

Key responsibilities include completing Care Act assessments, devising care plans, conducting reviews, Mental Capacity Assessments, participating in Best Interests meetings, and ensuring Bolton residents receive appropriate support.

You will work with resources from Social Care, Health, and voluntary/statutory agencies to assess individuals' independence and determine their ongoing support needs. The role also involves administrative duties, managing a busy duty service, taking referrals, and providing advice to the public and professionals.

We need resilient practitioners who can prioritise effectively, make confident decisions and remain calm under pressure.

While in this role you will need to demonstrate commitment and enthusiasm to working closely with those with care and support needs, their families and carers, other professionals and your managers and peers. You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and decision making.
