Full Job Description

An exciting opportunity has arisen to join West Cheshire Community Learning Disability Team as a Community Practitioner Assistant (Supporting Speech and Language Therapy and Occupational Therapy). You will have the opportunity to work with people in their own home implementing innovative therapy programmes, making a real difference to the lives of people with learning disabilities. The work could include helping people live more independently, supporting people with visual information or training staff on how to alter food to different consistencies to help someone swallow safely.

The team is a supportive multidisciplinary team made up of SALT, OT, Psychology, Psychiatry, and Learning Disability Nursing and you will receive regular support and supervision from SALT and OT. The Trust is very committed to staff training, and you will have access to a range of training opportunities as part of the heart of care programme and other role specific training.

We welcome applications from people with a variety of backgrounds. Ideally experience of working in a face to face role with people who have a learning disability, which may include adults or children, and other possible conditions they may have.

There is an expectation that you would cover the West Cheshire area and this involves travelling to people's homes and also work into two different offices, currently in Ellesmere Port and Winsford. The base for the successful applicant will be agreed as part of the recruitment process.

Main duties of the job

To work as an autonomous practitioner but with clear supervision from therapists.

To have involvement in assessments, treatment plans and training for people who access the service, which could include communication, dysphagia, sensory processing, meaningful activities, total communication.

A large portion of your time will be working face to face with service users in a variety of different environments, including a person's home, day service and hospital setting.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
