+ To provide an evidence-based phase I-III cardiac rehabilitation in line with all national and local guidelines.
+ To take an active part in the exercise and secondary prevention components of the cardiac rehabilitation service held at multiple sites in the Medway area.
+ Under the guidance of specialist nurse assist in answering correspondence and telephone calls from cardiology patients, to provide advice, support and follow up.
+ To visit cardiology patients in their homes or in clinic to review their progress, facilitate medication optimisation and support lifestyle changes.
+ To contribute actively to the monitoring and audit of the cardiac rehabilitation service., To work under instruction from the senior staff to:

An exciting opportunity has arisen in the community cardiac rehabilitation team. We are particularly looking for someone with either community or cardiology experience.
The role would suit someone with excellent communication skills, who is keen provide education and motivate patients in their rehabilitation.

· We will offer you the choice of two pension schemes; the NHS Pension scheme and the scottish widows group pension scheme.
· MCH encourages all staff to be double COVID vaccinated to protect themselves, their colleagues, their family and their patients.
