Full Job Description

We are seeking a highly organised and motivated administrator to join our established Care Navigator team in Nottinghamshire Healthcare NHS Trust. The post involves the organisation of multi disciplinary team meetings, case finding using e-Healthscope data and helping to co-ordinate care for complex and vulnerable patient groups. We are looking for someone who enjoys working within a team and making a difference to patient's lives. You will work closely with primary and community healthcare teams in the Byron (Hucknall) locality , alongside linking with with social care, mental health teams and voluntary organisations. This is an excellent opportunity for the post holder to use their administrative and organisational skills to help clinicians deliver excellent patient care., You will be based in Hucknall. We welcome applications for flexible working for the 25.5 hours to be worked over the week. However, the agreed hours must cover working Mondays, Tuesdays and Wednesdays and flex according to need as agreed with line manager.
