Full Job Description

To undertake holistic needs-led assessments in partnership with service users, patients and carers, prepare care plans, commission services and manage a caseload to ensure the most effective and beneficial care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements, Torbay and South Devon NHS Foundation Trust policies, standards and guidelines.

  • PLEASE NOTE: This post is not eligible for sponsorship in line with the Home Office standards., + To be a core member of a multi-disciplinary Baywide Health, Wellbeing and Specialist Services Team and contribute to the effective assessment and discharge planning with working age adults in the area of Torbay and for Torbay residents in out of area hospitals in the region
  • + To undertake support planning for individuals with a learning disability or neurodiversity + To undertake holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust policies and guidelines + To have clear responsibilities for the safeguarding of adults and children and for operating in a manner consistent with Human Rights legislation + To be compliant with and practice utilising social care legislation including Care Act 2014 and Mental Capacity Act 2005, Assessments are undertaken within a multi-disciplinary setting, and reflect the perspective of the individual and their carer, and lead to care plans which promote maximum independence and promote outcomes which are agreed with the individual who is being assessed To be responsible for managing and prioritising a high volume personal caseload of varying complexity Commissions health and social care services to meet individual assessed needs in care plans within budget. Inform clients of charging policies and the role undertaken by the Financial Assessment and Benefit Team To maintain accurate and up to date case records, and timely completion of care management forms as required by data management systems (PARIS). To take notes at case conferences and strategy meetings for vulnerable adults where an Administrator is unavailable and where the post holder is the care manager. To complete manual and IT-based records which meet Torbay and South Devon NHS Foundation Trust quality standards. Contribute to the collation of information needs within the community to assist the development of appropriate resources. Contribute to the decision-making on client priority needs. To participate in shared team responsibilities, and provide cover for absent colleagues according to the team's requirements. Where appropriate and having applied Care Act 2014 eligibility criteria, arrange services to meet the individual's assessed needs, within the framework of the social care contracting process. In accordance with Torbay and South Devon NHS Foundation Trust policies, provide information and advice on alternative services which may be available. To demonstrate an ability to undertake duties with greater autonomy and accountability, and require less direction and direct supervision. To act as a Duty Officer dealing with emergencies and same day urgent assessments as required. You will be joining the organisation at an exciting time.
