Full Job Description

  • The post holder will be responsible for the on-going assessment of care needs and the development, implementation and evaluation of programmes of care.

  • To be involved in discharge planning meetings with support from other team members informing the meetings of the role of the CCNT and what is expected of the family and what can be provided by the team.

  • To establish or maintain relationships with the child and family where relevant whilst the child is an inpatient in hospital to help facilitate smooth discharge.

  • To advise encourage, support and monitor the care being undertaken at home. If the child has equipment being provided by the Team to ensure that it is regularly serviced and maintained.

  • The post holder will take responsibility for ensuring adequate supplies for a delegated caseload of children who require equipment and consumables within the home.

  • To ensure that care plan and assessments are kept up to date and any changes recorded in a timely manner.

  • To inform team leader or in the absence of them a band 6 of any concerns related to the care in the home or school.

  • The post holder is responsible for maintaining their own knowledge and competencies in accordance with Trust policies and the NMC code of professionals conduct.

  • To provide accurate patient records of all aspects of care of children in order to promote continuity of care.

  • Liaise relevant information with the Hackney Ark MDT and other professional's involved in the care of the child.

    We are looking for an, enthusiastic and highly motivated registered children's nurse to join our team. You will be a RSCN/RN (Child) and will be responsible for delivering a high standard of nursing care. You will have excellent communication skills and the ability to work using your own initiative as well as working effectively within a team.


  • You will understand the importance of patient care and be able to demonstrate ability in the assessment, planning, provision and evaluation of care of sick children and be able to make effective decisions in partnership with children, families & the multi-disciplinary team.

    You will be based within the Community Children's Nursing team based at hackney Ark. The team offer a 7-day service and you will be working within the generic caseload.
