Full Job Description

To work as a member of a team to provide support to families in the community with nursing need and provide training on specific nursing care to nurseries and schools and other settings as per individual care plan and becoming clinically competent at providing holistic care., To work in conjunction with the family, teachers, other carers and health care professionals to ensure a holistic approach to children.To join CCN's in visits to support assessment and deliver care andlearn specific nursing care and deliver this once competent to do so.

To undertake ordering of equipment when needed and actively contribute to the assessment, planning and organising of resources.Ensure the safety of the child by monitoring and reporting all concerns and changes in their condition to the named person accountable as per local guidelines.Work in partnership with the multidisciplinary team according to the care plan under the guidance and delegation of appropriate professionals.Be an effective communicator to ensure that a high standard of care is given to the child and family.

Maintain written clinical records using local documentation.Encourage and facilitate good practice by acting as a role model.

Undertake assessment and evaluation of care in partnership with the family under the direct/indirect supervision of the paediatric nurse, updating the home notes and protocols.
