Full Job Description
The Children's Community Nursing Team provides clinical nursing care, training and support to children, young people (aged 0-19) and their carer's at home or other community settings, enabling them to remain safely in the community and transition to adult services where appropriate.
Are you a highly motivated children's nurse with a with a proactive and dynamic approach to meet the needs of our children and their families? If so then submit an application! An exciting opportunity has arisen to join our friendly and progressive community children's nursing team for full time post as a band 6 nurse. The post sits in a well-established friendly CCN team that delivers acute, ambulatory and chronic, complex care providing a 7 day service to children and their families living in Hillingdon Borough. You will be required to work 1:4 weekends and some bank holidays. This challenging and rewarding role offers the opportunity to meet the needs of children, young people and their families within a multidisciplinary inter agency framework. We are looking for an enthusiastic forward-thinking children's nurse, community experience would be an advantage but not essential. You will be a highly motivated independent practitioner with sound evidence based skills. The post holder will manage a varied caseload, act as a resource for an identified area of clinical special interest and contribute to the on-going development of the CCN team.

You will be part of the Children's Community Nursing Team (CCNT) in Hillingdon, working closely with the children's nurses, school staff, other allied health professionals and medical staff at Hillingdon Hospital as well as the specialist dietitian.. You will also be part of a wider peer supervision group of children's nurses to support evidence based practice and innovation.
