Community Clinical Sister- Integrated Front Door Nurses

You will need to be able to work in an autonomous way and facilitate effective communication between ward staff , the integrated discharge team and social care team, to promote a culture where transfers of care between primary, community and secondary services are safe, timely and well co-ordinated for patients who no longer require acute hospital care. Part of the role will involve working within the Community nursing teams proactively supporting and case managing a caseload of complex patients with a range of long term conditions, who are at high risk of unplanned admission to hospital or prone to unnecessarily long lengths of stay.

• You must be able to travel throughout the working day across the borough.
