Full Job Description

Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues. Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services - and to prevent them becoming disenfranchised and isolated., Diverse Cymru will start to deliver this new Community Connectors service for the 18 GP practices in Cardiff East, Cardiff South East and Cardiff City and South Clusters in Autumn 2024. This project provides an excellent opportunity to make a difference to people within three of the most deprived Primary Care Clusters in Cardiff and the Vale of Glamorgan, providing a mechanism to signpost and direct identified adult patients (over the age of 18), to appropriate community support groups and networks that are specific to their social and wellbeing needs.
It will be important that as a Community Connector, you are rooted in the local communities, have existing or are able to develop good contacts to local support services, community, and peer support groups and that the cultural needs of patients and communities are understood and always served.
