Full Job Description

The Forensic Intensive Recovery Support Team (FIRST) has an exciting vacancy for the position of Community Forensic Mental Health Nurse within our Multi- disciplinary community team. We are seeking an experienced and enthusiastic nurse to join our expanding team and work within the Forensic Intensive Recovery and Support Team (FIRST). We are looking for a Nurse who is passionate about making a difference to the lives of people who have experienced forensic inpatient services and is committed to the development and delivery of a high quality, service and discharge pathway. FIRST work closely with other forensic services in the West Midlands, as part of the REACH OUT partnership, a new provider collaborative. MPUFT's FIRST support men and women from the northern West Midlands in the process of discharge from forensic inpatient services and work to ensure safe, effective and successful transitions to the community. The post holder will be part of a multi-disciplinary service supporting transition of patient care from secure settings into the community. Participation in an out of hours on call rota, to support crisis responses may be required., The post holder will be required to work collaboratively with service users, carers, family and friends and housing providers providing support, and spending therapeutic time with an allocated group of service users with complex mental health and social needs, often with histories of offending and/or risk behaviours, both individually and within group settings. They will be responsible for care coordination of an identified caseload, be required to promote service-users recovery and transition process from an inpatient setting and to assist people in maintaining their place in the community and to assess, plan, implement and evaluate individualised care/recovery plans, using a variety of appropriate clinical and risk assessments. A key role will be supporting the transition out of FIRST into secondary mental health services. The post holder will also be expected to be available for consultation, support and clinical supervision to other health professionals working within the inpatient forensic service, Community Mental Health Pathway Service and other areas of the Trust where identified as appropriate. As the team expands and develops the post holder may be required to help provide provision of a 24-hour, 7 day per week crisis intervention service to cover community-based service users within the forensic directorate.
