Full Job Description

The successful applicant will be caring for patients with cardiac conditions such as coronary heart disease (CHD) and heart failure (HF). They will be expected to support the Heart Failure Specialist Nurses team in providing care for these patients by recording observations such as heart rate, BP and pulse oximetry and phlebotomy.

They may be directly involved in delivering cardiac rehabilitation, to both CHD and HF patients, alongside the exercise facilitators and cardiovascular nurses.

This post offers an exciting opportunity for a health care assistant to join the Specialist Coronary Heart Disease & Heart Failure Team based at the Greenhouse in Annfield Plain. The post covers core hours of 09:00-17:00 Monday-Friday with currently no weekend working although that may change in the future., If you are being interviewed you must accept an interview slot in the system to continue, even if you have arranged with the manager

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
