Full Job Description

We are seeking an enthusiastic, motivated, reliable Community Health Care Assistant who is keen to support and deliver a range of high quality nursing and phlebotomy services to our patients based in Chester., Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital.

As the role involves visiting patients in their own environment, access to transport for work purposes is essential.

An enthusiasm to support our team to deliver high quality services is a must for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable and approachable.

We are a very friendly, compassionate and highly motivated team that work hard to deliver the best outcomes possible for our patients and are seeking someone with similar values to come and join us.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
