Full Job Description

We step in to support people who are discharged from acute hospitals and those who need an urgent community response. The person needs short term/interim support in their homes. The teams are committed to caring and supporting people at home and work over a seven day rota.

What are the hours?
Various shift patterns
(Including unsocial hours, weekends and Bank Holidays)
Where would I be working?
Base: Florence Nightingale Community Hospital
Area Covered: Derby City and surrounding area
Working in this team means you will need be able to meet the travel requirements of going to people's homes. This will require you have a UK driving license and access to a vehicle. DCHS has a mileage policy and car lease scheme available to support our staff., The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual care without direct supervision in the community.
To provide support to patients to optimise their health and independence.
Be willing to support Perth House in Chaddesden when and if staffing cover requires.
