Full Job Description

The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.
To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.
To carry out individual care without direct supervision in the Perth House Assessment Hub.
To provide support to patients to optimise their health and independence.

You will be working a 4 on 4 off rota pattern, consisting of morning shifts 7.15am - 14.45pm and evening shifts 14.30pm - 22.00pm
The rota pattern will include weekends and bank holidays.
Where would I be working?
Base: Perth House Assessment Hub, Athlone Close, Chaddesden, Derby. DE21 4BP., Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
