Community Healthcare Assistant

This is a newly created post to join our community nursing teams in Wymondham and Attleborough as a Community Healthcare Assistant.

The role will be to visit patients in care homes across the "Ketts" area, and address a variety of care needs, for example,diabetes care / insulin administration, wound care, venepuncture and catheter care. Our service works 7 days a week. You will be able to demonstrate flexibility as well a sound clinical basis and understanding of community working. We would welcome prospective applicants to contact us for further information as well as meeting the team. You will be able to demonstrate a good basic knowledge of health care and appreciation of community based working., 1. To contribute to the delivery of a variety of interventions for patients with long term conditions, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible. 2. To work within the integrated team to facilitate early discharge from hospital 3. To work within the integrated team to prevent unnecessary admission to hospital 4. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
