Full Job Description

We are looking for enthusiastic a Community Healthcare Support Worker to provide care and support for patients in the community setting as delegated by the registered nurse based in Teddington working with the Teddington and Hampton team.
Access to a motor vehicle is essential - car, with appropriate driving license. Please ensure that your application demonstrates that you meet the essential criteria as listed in the job specification.
There may be an opportunity for the successful candidate to transfer onto the Trust's apprenticeship scheme and develop as a Registered Nurse Associate., To work within the Community Nursing Team to provide care and support for patients in the community setting as delegated by the registered nurse.
To carry out clinical duties and be prepared to undertake clinical skills update as per needs of the service and specific patient after training and competence assessment, with support from an identified mentor for example, venepuncture, blood pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and noncomplex assessments such as continence reviews.
This will involve working independently but as part of the Community Nursing Team covering a specific geographical area and be flexible to support corporate approach to patient care depending on the need of the service.
