Full Job Description

To provide a supporting role to registered practitioners in the Integrated Community Team in the delivery of nursing and rehabilitative care respecting patients' privacy and dignity at all times, To assist in planning, delivering and evaluating programmes of care and rehabilitation prescribed by the registered practitioners, regularly liaising at the beginning and throughout each shift.

We are looking for enthusiastic individuals to fill our band 3 Health Care Support Worker role to work in our mixed professional team. This will involve caring for patients in their own homes and seeing patients in the wound clinic. You will be committed to working within a generic competency framework that enable you to have the skills to work as a valued member of the team.

Applicants must have or willingness to be working towards a NVQ Level 3

Access to a vehicle and possession of a full driving license is essential.

Community Experience is preferred but not essential. All support and training will be given to the right applicant.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
