If the answer is yes, then this exciting opportunity to join our county wide Community Heart Failure Service at Midlands Partnership Foundation NHS Trust could be the perfect career move for you., As a Registered Nurse with current NMC registration you will have significant post qualification experience in the field of heart failure. With strong communication and organisational skills and a flexible approach, you will work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes.
You will demonstrate the ability to effectively and efficiently assess, plan, provide and evaluate evidence based nursing care for patients.
You will demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.
You will have a commitment to professional development and contribute to a culture of continuous improvement.
You will organise and prioritise your own day- to-day work tasks and activities and delegate tasks to junior colleagues.

· Improvement in health and wellbeing including mental health
· Interventions in disease/condition management
· Prevention and reducing of health inequalities
· Acute hospital admission avoidance
· Early supported discharge
· Case management and treatment /care plans
· Specialised interventions around specific long term conditions and clinical need
· Palliative and End of Life
· Improvement in health and wellbeing including mental health
· Ability to work without supervision or direction
5. To use professional knowledge and judgement to take into account the range of care/support packages and treatment options available. To consider patient's circumstances and preferences in order to implement and adjust programmes of care and intervention
