Full Job Description

To provide a high quality, comprehensive Information, Advice and Signposting to Community Hub customers.

Supporting the triage of referrals, identification of desired customer outcomes and support these to be achieved, though advice and information and signposting. Where required carrying out Screening Assessments and determining eligibility for financial support.

PRINCIPAL ACCOUNTABILITIES

1 To be responsible for receiving and resolving, where appropriate, customer contacts.

2 To identify a customers enquiry promptly and proactively and assess the appropriate response.

3 To carry out Screening Assessments, determining customer outcomes and meeting these need.

4 To support the identification of Safeguarding alerts and use relevant procedures to ensure they are managed appropriately (gaining immediate Operational Manager advice).

5 To develop a plan in response to the person's assessed eligible needs and decide and arrange where appropriate the provision of specific services.

6 To determine if service users require further assessment / redirecting to other services, including Adult Social Care services.

7 To undertake administrative work and ensure information is input on the client database to ensure accurate case records are maintained.

8 To ensure that service information statistics are recorded in order that facilities can be monitored and reviewed.

9 To attend regular meetings for the discussion of work policies and processes and act in line with these to ensure consistency and a high quality of service delivery.

10 To participate in regular training and development activities and to actively expand and update knowledge of Council and all other relevant services.

11 You must be prepared to implement the Council's Equality's Policy at a level appropriate to the job and must at all times carry out your duties with due regard to the Council's Equality's Policy.

12 You must be prepared to be responsible for the implementation of, and compliance with, the provisions of legislation relating to the health and safety of such employees and areas of the workplace as fall under your direct control and for complying with legislation relating to such works and contracts as are within your direct responsibility.
