Full Job Description

Are you a dynamic and highly motivated nurse ready to make a difference in patient care? Join our progressive and supportive Warrington & Halton OPAT/IV service, where you'll play a pivotal role in transforming the way care is delivered in the community.

As an OPAT/IV Nurse, you'll work alongside an expert team to provide cutting-edge, patient-centred care. This role offers you the chance to use your clinical skills to support early hospital discharges, prevent unnecessary admissions, and deliver exceptional care to patients needing IV antibiotics or long-term vascular access management.

Beyond patient care, youll collaborate with the OPAT team leaders to provide guidance, training, and advice to community nurses, secondary care teams, and GPs, making a lasting impact on healthcare delivery across the region.

Our service operates from 7am to 8pm, seven days a week, offering a flexible and rewarding environment where your skills and passion will shine. This is your opportunity to join an innovative team dedicated to improving outcomes and reshaping community healthcare.

Main duties of the job

Your key responsibilities will include

Performing peripheral cannulation, venepuncture, and port-a-cath procedures in line with established protocols.

Supporting district nurses by providing guidance and assessment in the administration of intravenous antibiotics.

Assisting with the delivery of intravenous therapy in patients homes, ensuring a safe and effective process.

Providing clinical advice and support to patients and district nursing teams during periods when the IV Team Leader is unavailable, such as during holidays or study leave.

Ensuring a smooth and seamless transition of care for patients by adopting a multi-disciplinary approach.

Delivering care in a clinic or patients home without direct supervision, while maintaining effective communication with the IV Team Leader.

Preparing detailed and accurate written updates on patients progress for review by the IV Team Leader.

Assisting in the development, implementation, and evaluation of patient care plans.

Supporting the education and skill development of community nurses to ensure competency in intravenous therapy.

Working within Trust policies and guidelines and following intravenous therapy patient care pathways.

Engaging with patients, families, and healthcare professionals through clear and effective communication.

This role offers an exciting opportunity to make a meaningful impact on patient care while fostering collaboration and professional development within the healthcare team.
