Full Job Description

The post is a 12 months fixed term position, created to cover a permanent staff member on secondment.

The post holder will be working as a member of the multidisciplinary health team in Brent. The team provides a specialist service to adults with learning disabilities in a range of community environments across the borough of Brent.

The post holder, as part of the Community Learning Disability Health Team is responsible for providing high standards of clinical assessment and intervention relevant to their specialist area. S/he will work with a range of other professionals, including physiotherapists, occupational therapists, psychologists, nurses, psychiatrists, social workers and a range of other carers and community staff. S/he may encounter a wide range of physically and emotionally challenging conditions in the clients' homes and other environments in the community. The post will involve working in a range of community environments including client's homes in Brent., 1. Provide appropriate support and contact for service users, taking into account their health, social and cultural needs, through thorough assessment and planning of the client's health needs.
2. Ensure that clients are consulted about their care at all times and are involved in decision-making concerning their care.
3. Carry out risk assessments for clients in line with the Trust and other relevant policies and procedures.
4. Develop and maintain innovative and creative ways of working with clients who are difficult to engage.
5. To provide clinical supervision to junior staff and students.
6. Ensure that accurate clinical notes are kept and audited to ensure compliance with records keeping standards of the Trust and LD Service.
7. Take responsibility for ensuring that clinical practices are in keeping with relevant legislations and policies, and follow best practice guidelines.
8. Be professionally accountable for own practice to your head of profession. Ensure that service users and their families are fully involved in the provision of care and services.
