Are you looking for a new challenge that will develop your clinical, leadership and team-working skills?, The post holder will work within the Community Matron Team to proactively manage, co-ordinate and support an identified caseload of patients with multiple complex long-term conditions, with responsibility for ensuring all health and social care needs are met to improve health outcomes, functional status and quality of life, ensure integration of all elements of care, increase patient choice and prevent unnecessary admissions to hospital and reduce the stay of necessary hospital admission.

We are looking for highly skilled, enthusiastic, self-motivated senior nurse to join our Community Matron Team based at Clarendon, Hebburn.

The role of the Community Matron is an essential part of the Integrated Community Team - working closely with GPs in the locality.

We want to recruit senior clinicians who are able to support and empower patients to manage their long term conditions whilst working to achieve realistic goals that are patient-led., You will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnoses; start, plan and evaluate care. You will also have organisational and time management skills. You will demonstrate leadership skills across the inter-professional team.
