Full Job Description

We have two exceptional opportunities for experienced healthcare professionals to join our Community Nursing Teams in Bracknell and Wokingham asCommunity Matrons. As a key members of an integrated community nursing team, you will play a pivotal role in managing a complex caseload of high-risk patients. Using your advanced clinical skills and clinical reasoning, you will deliver high-quality patient-centred care. You will work with patients to promote shared decision making regarding their care plans, encourage them to make informed decisions about their health and to self- manage long-term conditions with the aim to prevent hospital admissions. This is a full-time role within a service that operates 7 days a week. The position includes a requirement to work some late shifts, one weekend every eight weeks and rotational bank holidays.   • Working comprehensively to manage complex patients within the community nursing team and alongside key stakeholders, contributing to improved health outcomes and enhanced quality of life for patients with multiple chronic conditions.
  • Take a leading role in the care of patients with complex needs, mentor junior team members, and support the team with scheduled caseload management.
  • As the senior nurse, support the community nursing team using independent prescribing and advanced assessment skills to provide expert care whilst supporting scheduled caseloads, it is therefore essential that you hold a Nurse Independent and Supplementary Prescriber Qualification.
  • You will need to hold a valid UK driving license and have access to a vehicle suitable for work purposes or be able to travel as needed to meet the demands of the service. Please note that some office locations and client sites may not be accessible by public transport.
